WW High-Grade Colposcopy Central Referral Program

Waterloo Wellington Regional Cancer Program has partnered with WRHN @ Midtown, WRHN @ Queen’s Blvd SCOPE program, and Amplify Care to develop a central referral program. The goal is to expedite information transfer and shorten the wait time for high-grade cytology to Colposcopists in our region.

• The referrer (i.e., primary care providers, gynaecologist offices who do not perform colposcopy procedures, midwives, walk-in clinics) can send a referral through Ocean eReferral (search for Waterloo Wellington High-Grade Colposcopy Central Referral Program).

• Alternatively, a referral form can be faxed.

• A SCOPE Nurse Navigator will identify the next available Colposcopist and notify the Colposcopist office staff of the pending referral.
• The Colposcopist office staff will schedule the patient’s appointment based on the information provided by the SCOPE nurse navigator.

The patient will be called and provided with information by the Colposcopist office staff. Staff will share information about how to prepare for the procedure along with details about the date, time and location of the patient’s colposcopy. Email notification will be sent to the referrer and patient (if email is on file) with appointment details via eReferral. In case of fax referral, the Colposcopist office staff will fax the appointment confirmation to the nurse navigator, who will inform the referrer.
