
As a patient on active cancer treatment with the Waterloo Wellington Regional Cancer Program, you have access to a free, after-hours nursing telephone service called CareChart.
CareChart Digital Health is a virtual nursing program that can help you, and your care parter/family, manage your cancer and cancer treatment related symptoms after-hours.
The goal of the CareChart program is:
A nursing note with details about your call will be sent to your cancer care team after you talk to the CareChart nurse. Your CareChart nurse may ask you to follow up with your cancer clinic nurse the next day.

– An operator will answer your call and connect you to a CareChart specialized oncology nurse.
– You should expect to speak with a nurse within 15 minutes.

-Through the app, use video or voice calls to connect with a CareChart specialized oncology nurse. The app allows you to enter information about your symptoms. The CareChart nurse can review the information you enter, which may speed up your call.
Call CareChart only when your cancer clinic is closed. This includes evenings, weekends, and statutory holidays. During regular clinic hours, you can contact your cancer clinic directly.
After-hours services are only available when WRHN Cancer Centre is closed. See the hours of operation below:

Closed on Saturday, Sunday, and Statutory Holidays

During the call, the CareChart nurse will ask for information about your diagnosis, your treatment, the date of your last treatment, and other important details about your care (like symptoms or recent changes in health).
