Goals For My Cancer Care

Important Goals

What is most important to me? What gives my life meaning? What do I hope treatment will do for me?

Here are a few common examples, select any that apply:

  • Able to eat and taste food
  • Religious/Spiritual/Cultural beliefs
  • Achieve a particular life goal (wedding, milestone, etc.)
  • Improve or maintain my quality of life
  • Be at home
  • Prolong life
  • Be aware
  • Not burden others
  • Maintain independence
  • Be physically comfortable
  • Provide support for family
  • Other:

Fears and Worries

What fears or worries do I have about my health, care, or treatment options?

Here are a few common examples, select any that apply:

  • Ability to care for others
  • Getting unwanted treatments
  • Burdening others
  • Loss of control
  • Concerns about the meaning of life
  • Loss of dignity
  • Death or dying process
  • Symptoms or side-effects
  • Emotional/spiritual distress
  • Finances

Function and Abilities

What is most important in my daily life? What does a good day look like? What would be most difficult to lose?

Here are a few examples, select any that apply:

  • Activities of daily life (dressing, bathing, eating, etc.)
  • Travelling
  • Being without pain or discomfort

  • Work
  • Interacting with others
