Fill out symptom screening online before your in-person, phone or video appointment. Talk to your team about symptoms that concern you.

Symptom screening helps you and your healthcare team to:

  • Talk about symptoms that are important to you
  • See any physical symptoms and emotional changes change over time
  • Decide how to manage your symptoms

Your Symptoms Matter (YSM) is a good way to report cancer symptoms. Many appointments are by phone or video. Filling out YSM gives us a full picture of your health before your appointment.

How to do ‘Your Symptoms Matter’ at home

Your symptom report is only checked during your appointment. If you have urgent symptoms:

1 day before your appointment: 

Scan the QR code

OR

Go to isaac.ontariohealth.ca

Enter your OHIP number and click ‘start’. Answer the questions and click ‘continue’
