On February 6, 2026, Waterloo Regional Health Network (WRHN) was notified by OntarioMD of a security incident that impacted WRHN’s connection to the Health Report Manager (HRM) tool. The HRM is a secure tool hosted by Ontario Health commonly used to send information about a patient’s care in the hospital with their primary care provider.
The incident was contained immediately upon discovery and occurred outside Ontario Health’s, OntarioMD’s and WRHN’s networks. WRHN’s internal information systems were not affected and remain secure. This incident did not impact any other healthcare providers or hospital organizations.
While it has not been determined whether any personal health information (PHI) was impacted, a formal investigation is currently underway. The investigation may take a month or more to complete. As a precautionary measure, WRHN, OntarioMD and Ontario Health have notified the Information and Privacy Commissioner of Ontario about the incident.
If it is determined that any Personal Health Information (PHI) was accessed by an unauthorized user, WRHN will notify affected patients directly and provide additional information.
