Seamless Care Optimizing the Patient Experience (SCOPE)

Waterloo Regional Health Network (WRHN) is committed to making it easier for patients to get the right care at the right time. Our work with the SCOPE program brings us closer to that goal.

SCOPE helps primary care providers, hospitals, and community health teams work together to support patients with complex health needs. Our nurse navigators help providers find the right care for their patients more quickly and easily.

Benefits of SCOPE

Primary care providers support patients who often have complex or long-term health needs.

When providers have to spend time searching for the right resources, they have less time to care for their patients. This means patients may have to wait longer for care. By using SCOPE, providers can spend their time and energy on what matters most — giving patients the best care possible.

SCOPE Pathways

SCOPE pathways help providers find the right services for their patients.

Current pathways include:

  • general internal medicine
  • heart failure/dyspnea
  • urgent imaging
  • iron deficiency anemia (non-pregnant adults)
  • Waterloo Wellington High-Grade Colposcopy Central Referral Program
