My Connected Care is a free, secure online tool. It gives patients of Waterloo Regional Health Network (WRHN) access to their health records and personal health information.
With My Connected Care, you can:

Anyone who is age 12 and older and is a past or current patient at WRHN can have an account.
When you sign up for My Connected Care, only you can see your health records. If you want to give another person access your records, contact WRHN’s Health Records department.
The My Connected Care patient portal is a WRHN initiative. The information on the portal reflects the shared electronic health record used across WRHN sites.
The portal has security measures to protect against risks, such as accessing, collecting, using,
disclosing, or disposing of information without permission.
