Referrals to the KW4 Prenatal Clinic are accepted through our obstetric roster program. More information about the KW4 Prenatal Clinic is available on the KW4 OHT website.
The KW4 Prenatal Clinic provides early prenatal care for patients who do not currently have a primary care provider. Providers can refer patients to the clinic by submitting a referral to the obstetric roster at WRHN.
Eligible patients will be connected to the clinic for early prenatal care. Once patients reach their third trimester, they will transition to an obstetrician for ongoing care and delivery.
This service is for expectant patients who need access to an obstetric provider at WRHN @ Midtown. Patients must reside in Kitchener, Waterloo, Wellesley, Wilmot, or Woolwich to be accepted for roster.
Please include the required test results and documentation listed on the form with your referral.
