The Complex Continuing Care Team at Waterloo Regional Health Network (WRHN) is here to support you and your family. Whether you are living with a long-term illness or working to regain your strength and independence, our team will help you live as safely and comfortably as possible.

We care for people who need ongoing support for long-term health conditions. We also offer rehabilitation therapy to help you recover after a health event. Our services include:

  • care for changes in memory, thinking, and behaviour
  • assessments for older adults
  • complex medical care and breathing support (for patients who need a ventilator)

A team of doctors, nurses, therapists, and other health professionals will work together to meet your unique needs. Our goal is to support your health, comfort, and quality of life in a safe and respectful environment. You will need a referral from a doctor or nurse practitioner to get services.
