At Waterloo Regional Health Network (WRHN), My Connected Care helps you manage your health online. Stay informed, feel supported, and get important updates from your care team, all in one place.

When you are a patient at WRHN, you can use My Connected Care to see your health records online. This includes your test results, clinical notes from your healthcare team, and other health information. It is a free, secure tool you can use on any computer or device web browser.

My Connected Care lets you view most health information shortly after it becomes available in our system, including:

  • viewing laboratory reports (three-hour delay)
  • viewing diagnostic reports (three-day delay)
  • reading clinical notes written by your healthcare team
  • viewing upcoming appointments
  • viewing messages from your healthcare team
  • completing questionnaires before your appointment
  • viewing allergy information
  • viewing pathology reports (10-day delay)

My Connected Care Introduction for Patients at WRHN

Create an Account

To sign up for My Connected Care, visit the self-enrollment page. You will need to include your first and last name, your date of birth, and one of the following:

  • your health card number and medical record number (MRN), or
  • the email address we have on file for you.

You can find your MRN on documents from WRHN. These include appointment notices, armbands, or discharge summaries.

If you do not know your MRN or do not have an email address in your medical record, you can fill out the self-enrollment assistance form. We will confirm your information, and a team member will send you a secure link by email so you can finish signing up.

Giving Others Access to Your Health Information

An authorized representative is someone who has permission to view another person’s health information in My Connected Care. Authorized representatives can be:

  • parents and legal guardians of patients under 12 years old
  • caregivers for patients who cannot make their own health decisions
  • people patients choose to give access to

Removing Authorized Representative Access

If you are 12 years old or older, you can choose to remove other people’s access to your account at any time. To do this call the 24/7 My Connected Care customer support line at 1-855-455-2717. This is a toll-free number to the Cerner Customer Care Support Centre, located in Kansas City, Mo.  

When you remove an authorized representative from your account, they will no longer be able to view any of your health information.
